Generation Patient is thrilled to be hosting a Town Hall on Young Adults with Chronic Conditions Leading Health Systems Change on Thursday, April 23, from 12-1, Eastern, and we want to invite you to attend (and if you can, spread the word)! 
For the past year, Generation Patient has hosted a Roundtable Series supported by the Lucile Packard Foundation for Children’s Health to explore the most pressing systemic challenges facing young adults with chronic and rare conditions. Bringing together young adult patients, clinicians, social workers, care partners, researchers, and policy experts, participants explored barriers in the healthcare system for young adult patients and identified opportunities for meaningful change. Each roundtable, which explored topics including medical trauma, peer support, and the role of artificial intelligence in care, has resulted in a peer-reviewed publication that will capture insights and recommendations, with the perspectives of young adult patients at the forefront.

On April 23rd, we will share more about the roundtable series, including:
  1. How the Roundtable model can be used for multidisciplinary collaboration
  2. What key themes emerged across the series
  3. How young adult patients are supported as lead authors on peer-reviewed publications
  4. How lived experience can drive systems-level change in healthcare and research

18th Annual HCTRC Research Symposium

October 21st, 2026

The International and Interdisciplinary Health Care Transition Research Consortium (HCTRC) in collaboration with the 27th Annual Chronic Illness and Disability Conference, Transition from Pediatric to Adult-based Care is pleased to announce the Call for Abstracts for researchers to be presented at the 18th Annual HCTRC Research Symposium in Houston, Texas, on Wednesday, October 21st, 2026. 

Our international panel of judges will review all HCTRC submissions. 

The categories of HCTRC abstracts for presentations (platform and poster) or breakout sessions include the following categories: 

  • Adolescent/Emerging Adult/Family Experience
  • Continuous Quality Improvement/Program Development/Models of Care
  • Health Provider or Patient/Caregiver Education
  • Health Care Transition Outcomes and Readiness
  • Self-Management

Gillette Children’s is hosting the 4th Annual Gillette Children’s Pediatric to Adult Health Care Transition Summit will take place Monday, May 18, 2026, from 9:00 a.m. - 4:00 p.m. at the Wilder Foundation. This hybrid Summit has AMA PRA Category 1 CreditsTM for CME. Credit amounts are to be determined. Register to attend in-person by May 4th! Register to attend virtually by May 11th!

The theme of the 4th Annual Health Care Transition Summit is Connection: Humanizing the Health Care Transition from Pediatric to Adult Serving Systems. We will bring together a statewide collaboration of advocacy, clinical, education, and policymaking experts to share best practices and exchange knowledge that successfully supports youth as they transition to adult services. The Summit will offer participants valuable insights on driving advancement in the field of Health Care Transition (HCT). Participants can use this knowledge to enhance their clinics, hospitals, programs, and overall state practices to support safe transition from pediatric to adult care systems.

Tentative agenda topics are:

  • Building Resilience through Connection during Healthcare Transition

  • Collaboration: Including Lived Experience in Building a Healthcare Transition program

  • Evaluating Family Engagement in System-Level Initiatives

  • Advocacy groups supporting youth/young adults and families during Health Care Transition

  • Presenting a Local Transition Program: Children’s MN Shift Program

  • Creating a Promising State Health Care Transition Initiative: The Rhode Island Story

We are delighted to welcome Dr. Patience White and Peggy McManus, from Got Transition, a program of the National Alliance to Advance Adolescent Health. Dr. White and Ms. McManus, along with clinical experts from Gillette Children’s, and other esteemed guests, will provide expertise and guidance to foster engaging dialogue centered on current conversations in pediatric to adult HCT.

Call for Papers

What Constitutes Successful
Health Care Transitions Outcomes?

 

Health Care Transitions is seeking papers that address issues associated with successful health care transition outcomes. We invite papers that explore this phenomenon from a myriad of perspectives: the pediatric/adolescent and adult-focused systems of care. We seek to papers that examine the biopsychosocial factors that lead to successful health care transition outcomes.

The guest editor is Dr. AnneLoes van Staa, professor of Transitions in Care at Research Centre Innovations in Care, Rotterdam University

We invite research, quality improvement systematic, integrative and scoping reviews, case studies, theory development, concept analysis, and evidence-based practice manuscripts. Specifically, we welcome submissions in the following areas:

  • HCT service models and programmatic services
  • Patient engagement
  • Outcomes/quality indicators in HCT
  • The health care provider role in HCT
  • Collaborative models of care
  • Clinical considerations with the transfer of care to adult-focused providers
  • Health care transition practice exemplars
  • Adult-focused care models serving adolescents and young adults who have transfers their care
  • Recommendations for improving research designs and methodologies
  • Recommendations to improve pediatric and adult-focused service model delivery

IMPORTANT NOTE: Health Care Transitions is a gold open access journal; the article processing charge has been discounted 50% for this special issue. See more information here. Corresponding authors are encouraged to check the list of participating OA agreements here.

Manuscripts should be submitted by December 1, 2025, to https://www.editorialmanager.com/hctj. The guide for authors should be closely followed and choose successful outcomes as the article type.
